Starting January 18th, 2022, we are enrolling all TK-10th grade students first come first serve until we fill our capacity.


1-You can come to our office to receive and complete the enrollment form.

 

2-You can download an Enrollment Form from our website and complete and send it with the documents.

 

3-You can complete an Online Enrollment Form, then we will contact you to complete the enrollment process.


Please bring the following documents to finalize your enrollment:

 

-Proof of birth: Birth Certificate, Baptismal Certificate, Passport (Please see AB 699 Compliance)
-Parent's photo ID: Guardianship documents if you are not the parent
-Proof of address: Utility bill (DWP, Gas)
-Physical Exam, vaccination record, and TB test
-Current report card
- A recent copy of IEP(if applicable)
-GATE program letter of the document(if applicable)

 

SHOTS REQUIRED FOR TK/K-12 STUDENTS

 

Students Starting 7th Grade Need:
Tetanus, Diphtheria, Pertussis (Tdap) —1 dose (Whooping cough booster usually given at 11 years and up)
•Varicella (Chickenpox) — 2 doses(Usually given at ages 12 months and 4-6 years)
•California schools are required to check immunization records for all new student admissions atTK/Kindergarten through 12th grade and all students advancing to 7th grade before entry. Parents mustshow their child's Immunization Record as proof of immunization.

AB 699 COMPLIANCE

 

Dear Parents/Guardians,
If for any reason you don’t have your child's Birth Certificate or don't want to disclose your child’s birth country, you will be able to provide that information later when completing the “Students Information Update Sheet” during the school year. Completing this information helps your child’s school secure additional funding to support its educational program.
